This excel based tool allows providers to collect pre/post and fidelity data for the LifeSkills Therapy program. The data tool for this project is more extensive and collects demographic data required for reports deliverable to Pennsylvania Commission on Crime and Delinquency grantees receiving technical assistance from EPIS Implementation Specialist.
